二术合剂穴位贴敷、拔罐联合足浴方对小儿胃肠功能紊乱症状改善及免疫功能、炎症状态的影响

摘要目的：观察二术合剂穴位贴敷、拔罐联合足浴方对小儿胃肠功能紊乱（脾虚证）症状改善、免疫功能、炎症状态的影响。方法：本研究为前瞻性研究。选取存在胃肠功能紊乱腹泻症状的患儿124例为研究对象,随机分为对照组和观察组,每组各62例。对照组患儿给予酪酸梭菌活菌散口服,必要时给予止呕、补液等常规处理;观察组患儿给予二术合剂穴位贴敷、拔罐、足浴方治疗,连续治疗7 d。对比两组方案的治疗效果、胃肠道症状分级评分（GSRS）,观察大便性状、次数改善情况,监测免疫功能相关指标（IgG、IgA、IgM、锌）、炎症状态指标（TNF-α、CRP、IL-17）水平变化。结果：对照组、观察组治疗有效率分别为88.71%、96.77%,有效率差异没有统计学意义;但按照不同的疗效等级进行秩和检验发现,以观察组的临床疗效更好;观察组患儿治疗后腹胀腹痛、恶心呕吐评分明显低于对照组;观察组患儿自第3 d开始大便性状较治疗前出现明显改善,逐渐趋于成形;且治疗第3 d、第7 d组间比较,以观察组的大便性状改善更明显,大便性状评分明显低于对照组;患儿治疗后大便次数有所减少,自第2 d开始大便次数明显减少;且同一时间点比较,以观察组的大便次数明显少于对照组;与治疗前相比,两组患者治疗后机体免疫力有所上升,炎症状态有所减轻,免疫球蛋白IgG、IgA、IgM水平及锌含量均明显升高,血清中TNF-α、CRP、IL-17水平均明显下降,以观察组各指标变化较对照组更明显。结论：二术合剂穴位贴敷、拔罐联合足浴方治疗小儿胃肠功能紊乱疗效确切,利于改善症状,可能与纠正患儿免疫功能紊乱状态、减轻机体炎症反应有关。

Abstract：ObjectiveTo observe the effects of erzhu heji acupoint sticking,cupping combined with foot bath prescription on the improvement of symptoms,immune function and inflammatory state of gastrointestinal dysfunction（spleen deficiency syndrome）in children.MethodsThis is a prospective study.A total of 124 children with diarrhea symptoms of gastrointestinal dysfunction were selected as research objects and randomly divided into control group and observation group,with 62 cases in each group. Control group was given live clostridium caseinate powder orally,when necessary,given antivomiting,rehydration and other conventional treatment;Observation group was treated with Erzhu mixture acupoint application,cupping and foot bath for 7 days. The therapeutic effect and gastrointestinal symptom grading score（GSRS）of the two groups were compared,the improvement of stool character and frequency was observed,and the changes of immune function related indexes（IgG,IgA,IgM,zinc）and inflammatory status indexes（TNF-α,CRP,IL-17）were monitored.ResultsThe effective rates of control group and observation group were 88.71% and 96.77%,respectively,with no statistical significance. However,the rank sum test according to different treatment levels showed that the clinical efficacy of the observation group was better,and the difference was statistically significant. The scores of abdominal distension,abdominal pain,nausea and vomiting in observation group were significantly lower than those in control group. In the observation group,the characteristics of stool were significantly improved from day 3,and tended to be formed gradually. In addition,the improvement of stool traits in the observation group was more obvious,and the score of stool traits was significantly lower than that in the control group. After treatment,the frequency of stool was reduced,and the frequency of stool was significantly reduced from the 2nd day. The number of stool in the observation group was significantly less than that in the control group at the same time point. Compared with before treatment,after treatment,the body immunity was increased,the inflammatory state was alleviated,the levels of immunoglobulin IgG,IgA,IgM and zinc contents were significantly increased,and the serum levels of TNF-α,CRP,IL-17 were significantly decreased. The changes of each index in the observation group were more obvious than those in the control group.ConclusionThe curative effect of the combination of acupoint application and cupping combined with foot bath prescription in the treatment of gastrointestinal dysfunction in children is definite,which is beneficial to improve the symptoms,and may be related to correcting the disorder of immune function and alleviating the body's inflammatory response.
